$10 Sports Physicals at the Summit Center

Presented by Northern Arizona Orthopaedics at Summit Center

One of the best, most convenient and affordable ways to get your kiddos all set for their sports activities in the school year ahead. Come early to get your spot and don’t forget on average, it takes about an hour to move through all the stations.

Northern Arizona Orthopaedics and DeRosa Physical Therapy will be hosting their annual Sports Physicals day. All student athletes who need to fulfill an annual pre-participation physical evaluation for the 2018-2019 school year are welcome! All schools are welcome! This awesome event only costs $10.

Things to note:

Please fill out the AIA paperwork BEFORE attending. Download Form 15.7A and 15.7B from Aiaonline.org.
All students <18 yrs. MUST be accompanied by a parent or guardian.
Doors will close at 11 a.m.
Please bring check or cash, no credit cards.
All funds from FHS students will be matched and will be donated back to Flagstaff High School’s Athletic Programs.

If you are short on time, we recommend getting here early and getting in line before the doors open. You may want to bring waters and layers, as it get's warm within each station. You will be on your feet most of the time, so dress comfortably.
