Northern Arizona Orthopaedics and DeRosa Physical Therapy will be hosting their annual Sports Physicals day. All student athletes who need to fulfill an annual pre-participation physical evaluation for the 2017– 2018 school year are welcome! All schools are welcome!

Things to note:

1. Please fill out the AIA paperwork BEFORE attending. Download Form 15.7A and 15.7B from Aiaonline.org.
2. All students <18 yrs. MUST be accompanied by a parent or guardian.
3. Doors will close at 11 a.m.
4. All funds from FHS students will be matched and will be donated back to Flagstaff High School's Athletic Programs.
